SCHMUCKER 8TH GRADERS – Do you want to join a Penn sports team in Fall 2016?
Before you can try out, practice or compete, the school must have an IHSAA pre-participation physical evaluation form completed and on file. The form includes a section for parents to complete and a section for a doctor/healthcare practitioner to complete.
- Penn High School Athletics will host athletic physicals for high school students and 8th graders (incoming freshman) Tuesday, May 24th from 3:30-5:00 p.m. in the Auxiliary Gym at Penn High School.
- The cost is $25 per student.
- Students and parents should complete the IHSAA physical form prior to arriving on May 24, with the exception of page 2 (doctor's portion).
If you choose to visit your own doctor, you will still need to have your doctor fill out the IHSAA form to take to your family doctor. To be valid for Fall 2016 sports, your physical exam must have been completed after April 1, 2016. Click here to download and print a copy or pick up a copy of the physical exam form at the Penn Athletics Office, located next to Door B. Once your doctor or other health practitioner has completed it, return it to the Penn High Athletic Office.
Prior to the second week of Fall 2016 practice, you will also need to complete, sign and return the following:
- Code of Conduct and Drug Testing Program Consent form
- Concussion and Sudden Cardiac Arrest Acknowledgement form
- Emergency Information form
- Athletic Accident Insurance ($50.00) – per student per year
- Transportation Fee ($10.00) – per student athlete per sport season
Coaches will have Code of Conduct packets available for their student-athletes, and packets are also available at the Athletics Office. The other required forms can be accessed at Penn’s Athletic webpage.
