The PHM Chromebook Insurance Plan covers 2 repairs over the course of one year. The cost is $25.

Please download the form, complete it, and mail it to Schmucker Middle School with the $25 payment.
Checks should be made payable to SMS.
Please include your student's PHM ID number on the check, if possible.
